The forms you need, without hunting for them.
Consent to share, service agreements, referrals and dashboard access. If you work with a participant we plan-manage, this is everything in one place.

Consent to share
Before we can talk to you about a participant's plan, they need to tell us that is alright. This is the form that does it, and it is the one people ring us about because it used to be buried.
Version 5.5, December 2025. PDF, opens in a new tab.
Who needs to sign it
The participant, or the person who makes decisions with them. Once it is with us we can discuss their plan with you directly rather than going back and forth through them.
Dashboard access
The dashboard shows a participant's providers, their services and what has been claimed, so you are not ringing us for a balance. Access is set up per coordinator once we have the participant's consent to share on file.
- Consent to share needs to be with us first.
- We set your access up against the participants you coordinate.
- Call us or ask your usual contact to arrange it.
Referrals and service agreements
If you have a participant who needs a plan manager, you can send them our way or start the conversation on their behalf.
Tell us about them
Call us or use the contact form. We will tell you straight away whether we can take them on.
They sign the service agreement
The agreement is between us and the participant. It lives on its own page so you can send them the link directly.
Consent to share comes with it
Once that is signed, we can deal with you directly on their plan rather than relaying everything through them.
